Parents/Guardians & Students, please have the student's birth certificate, any accompanying Affidavit/Amendments, and a pen to complete the CXC Registration.
​You may also take a picture of the completed registration page for your own records.
Parents/Guardians, please ensure you have a form of identification.

  • Form 5 from 8:30am
  • Form 6 from 12:30pm

Procedure for verification of CXC Registration by Parents

  • Parents/Guardians are to ensure:
    • the spelling of the student's name, date of birth, and Birth Certificate PIN is correct according to the Birth Certificate. If there is an issue, this should be verified against the Birth Certificate before any change is accepted, or an Affidavit/Amendment supplied.
    • the Gender (M or F) and Class (5-1, 5-2, 5-3, 5-4, 6L, 6U) is correct
    • the list of Subjects is correct (with Transfer, Resit, or Option where required)
    • the # subject is correct according to the list of Subjects
    • Parent/Guardian should the sign the page with their signature where indicated (bottom right of the page), write their name in block letters, relationship to student, date of verification, ID number, and type of ID
    • Parents/Guardian may take a picture of the page as students will need to know the Candidate No, and given username/password
  • If there are to be any corrections/additions, Parents/Guardians bring it to the attention of the person in charge.
  • The signed page should be returned to the Form Teacher
    • NOTE the page should not leave the school, therefore you are to submit the signed page before leaving.
