Form 1 Registration

Required documents for Form 1 Registration:

• Printout of Placement Slip
• Original AND a Copy of Student's Birth Certificate
• If applicable: Proof of Citizenship, if he/she is a national born outside of Trinidad and Tobago
• If applicable: Valid Student Permit and any relevant documents if he/she is a non-national
• Parents'/Guardians' Identification (National ID/ Passport/ Driver's permit) 
• If applicable: A Copy of Agreement or Limiting Order with respect to custody/shared parenting (in the case of divorce or separation of parents)
• Copy of Student Immunization Card
• Two (2) passport-sized Photos (with Student's Name written to the back)
• Completion of the online pre-registration form: http://erpstt.com/fass/staff/sea/

Please note that Student ID cards/Badges, Homework Notebook, Girls' ties, PE t-shirts
​and other items will be on sale at school.

Students are to be attired in full Primary School Uniform on the day of registration.

Note: Form 1 transfer requests are to be facilitated by MOE via District Offices.
Students MUST register at the school he/she was assigned then take transfer request form to the District Office.
